- Blue retirement application package
- Annuity Estimate (for the date you have chosen to retire)
Besides the two above, you need to bring the following:
- Latest pay stub (or recent)
- Banking information, routing number, account number, full mailing address of the bank and phone number
Please bring the following IF applicable:
- Marriage certificate, spouses date of birth, social security number, if married of course
- DD214 for enlistments or final retirement DD214 for retirement orders, if served in the military
If the following may apply, please call me first!
- Claim number and date of injury for Department of Labor Claim for serious injury and off work more than 45 days
- Divorce decree if divorce is after 1985
